How to Conduct Effective Workplace Risk Assessments

Why Risk Assessments Matter

A risk assessment is not a paper exercise. Done properly, it is the foundation of every decision you make about workplace safety: what controls to put in place, where to focus training, and how to demonstrate legal compliance to an inspector or auditor. Under frameworks like ISO 45001 and qualifications like NEBOSH, a structured, documented risk assessment is a core requirement, not optional.

This guide walks you through the process in plain terms, whether you are doing this for the first time or refreshing a procedure that has grown stale.

Step 1: Define the Scope

Before you identify a single hazard, decide what you are assessing. A whole facility is too broad to be useful. Break it into logical units: a specific work area, a task type (for example, working at height), a piece of equipment, or a process such as chemical handling.

Document who is affected. Include employees, contractors, visitors and, where relevant, members of the public. This scoping step saves time later and ensures your findings are actually actionable.

Step 2: Identify Hazards

A hazard is anything with the potential to cause harm. Walk the area physically rather than relying on memory or previous assessments. Talk to the people who do the work every day because they often know about risks that do not appear in any manual.

Common hazard categories to work through:

  • Physical: noise, vibration, extreme temperatures, moving machinery
  • Chemical: substances that can be inhaled, ingested or absorbed through skin
  • Biological: bacteria, viruses, mould (especially relevant in healthcare, food production and waste management)
  • Ergonomic: repetitive motion, awkward postures, manual handling loads
  • Psychosocial: workload, harassment, shift patterns that disrupt sleep
  • Electrical and fire: faulty wiring, ignition sources near flammables

Record each hazard as specifically as possible. “Chemicals” is too vague. “Exposure to hydrochloric acid during tank cleaning” gives you something to work with.

Step 3: Assess the Risk

Risk is a combination of two things: the likelihood that a hazard will cause harm, and the severity of that harm if it does. Most organisations use a simple matrix, rating each factor on a scale (for example, 1 to 5) and multiplying them to get a risk score.

A risk matrix helps you prioritise. A score of 20 out of 25 demands immediate action. A score of 2 can probably wait for the next scheduled review. Whatever scoring system you use, apply it consistently across assessments so comparisons are meaningful.

Also consider how many people are exposed and how often. A moderate hazard encountered by twenty workers eight hours a day carries more overall risk than a severe hazard that one person faces once a year.

Step 4: Apply the Hierarchy of Controls

The hierarchy of controls, recognised under ISO 45001 and most national safety regulations, tells you to work through control options in this order:

  • Elimination: remove the hazard entirely (the most effective option)
  • Substitution: replace a hazardous material or process with a less dangerous one
  • Engineering controls: physical changes such as guarding, ventilation or noise enclosures
  • Administrative controls: safe work procedures, training, job rotation, permit-to-work systems
  • Personal protective equipment (PPE): the last line of defence, not the first

PPE gets reached for too quickly because it is cheap and visible. That is a mistake. PPE fails, gets worn incorrectly, and does nothing to fix the underlying hazard. Use it to supplement higher-level controls, not replace them.

Step 5: Document and Communicate

Write your findings in a format that others can use. A good risk assessment record includes: the hazard identified, who is at risk, the current controls in place, the residual risk score, any additional controls required, the person responsible and a target date.

Share the findings with the people affected. A risk assessment that sits in a folder helps no one. Hold a brief toolbox talk, post a summary near the work area, or incorporate findings into induction training.

Step 6: Review and Update

Risk assessments are living documents. Review them after any incident or near-miss, whenever a process or piece of equipment changes, and at regular scheduled intervals (at least annually for most workplaces). Regulations and standards such as IOSH guidelines recommend more frequent reviews in high-risk environments like construction and oil and gas.

A risk assessment is only as good as the person conducting it. Supervisors who understand hazard recognition, risk scoring and control selection produce far more useful assessments than those working from a template they barely understand.
